Here at Marmalade Dentistry we strive to keep our carbon imprint to a minimum. Your new patient forms and releases will be sent Via Email for an easy at home “filling out your paperwork” experience. For those of you who are unable to fill out paperwork online we ask of you to come in 15 minutes early to fill out your online forms.
Below is our patient portal link for existing clients. This allows you to access to your health records, and appointment information including information about what to do after your appointment. Signing up is simple and only requires a few moments of your time.
When set your first appointment, we will ask for your Name, Phone number, Email, Birthday, and Zip code. These will all be needed for you to sign into your patient portal link.
We will then send you an email that will have a link in it for you to access our patient portal, and guide you through the set up process.
Once you have set a custom password for your account, that’s it! You will be all set up! You will now be able to fill out all of your paperwork for your appointment, and see a list of the appointments you have scheduled.
If you have not received a link to set up your patients portal please email firstname.lastname@example.org and we re- send you an invitation email.
If you ever lose the email that was sent to you, we have conveniently located a link down below to allow you to login from any computer.
